There have been discussions about this, but nothing official has been decided yet. However, HealthStream believes that basing payments on individual HCAHPS themes, rather than the Overall Rating, is where CMS may very well go. It is certainly something that hospitals and healthcare systems should have on their radar, as its implications could be far-reaching. One possible scenario could be that a hospital with stellar Overall Ratings but low Doctor ratings could get smaller reimbursements from CMS than a hospital with a mediocre Overall Rating but high theme scores. In this type of reimbursement environment, hospitals would need to strive for high ratings in all of the HCAHPS themes.
