The challenge to constantly improve the safety and quality of healthcare has always been an inherent responsibility of those who deliver care. It is the duty of providers to ensure that people receiving their care are kept free from danger or risk of injury while receiving that care. Since the early 1990s, however, research studies conducted respectively in the United States, Canada, Great Britain, and Australia have found that between 4 percent and 16.6 percent of patients suffer from some kind of harm (including permanent disability and death) as a result of adverse events while in the hospital (Baker et al., 2004, Brennan et al., 1991) Department of Health, 2000, Kohn et al., 2000, Leape et al., 1995). These studies have also found that a significant percentage (around 50 percent) of the reported harms could have been prevented (Kohn et al., 2000, Leape, 1994).

The Delay in the July 2008 Hospital Compare Release
The July 17, 2008 release of Hospital Compare data for providers participating in the Hospital Quality Alliance (HQA) and/or the Reporting Hospital Quality Data for Annual Payment Update (RHQDAPU) program will occur later than previously announced. The QIO community and providers will be notified when Hospital Compare will be updated.

Still in Preview Period for the Fall Hospital Compare Data Release
The HQA Preview Reports will be available from July 16 through August 14, 2008. (To access the reports, you must be registered as a My QualityNet user and have been assigned the QIO Clinical Data Warehouse Feedback Report role.) The data that appear in the Preview Reports will be reported on Hospital Compare, the Centers for Medicare & Medicaid Services website for Medicare beneficiaries and the general public, tentatively in September 2008. For those hospitals that want to suppress reporting of results, you’ll need to file another suppression form on QualityNet, as forms are only valid for a quarter.

Next Data Submission Deadline Is October 8, 2008
We are currently uploading the results for patients discharged in April, May and June 2008. We will update our clients as uploads are all completed.

Spreading Best Practices: Holy Name Hospital’s Pediatric Fast Track Program

This Spreading Best Practices article will share the methodology that one community hospital used to expand their pediatric census, enhance family and physician satisfaction and provide additional opportunities to strengthen and grow the skill of the nursing staff. All details for Holy Name Hospital’s Pediatric Fast Track Program were provided by Sheryl Syby, RN, Clinical Coordinator of Pediatrics. Sheryl has been with Holy Name Hospital since 1988. She worked in the pediatric unit as a staff nurse until 2004, and was promoted to Clinical Coordinator of Pediatrics in 2005.

To assist the hospital in identifying its areas of strength, as well as its opportunities for improvement, HealthStream Research provides a leverage analysis. Targeting the opportunities identified by the leverage analysis, along with successful strategies for improvement, will result in the most significant increase in performance. The leverage analysis is a visual outline of the importance and performance of the survey items.
